Patient Information

Appointments should be scheduled as far in advance as possible. Every visit requires an appointment. Office telephone hours are 8:00am to 4:00pm Monday ­ Thursday and 8:00am to 1:30pm Friday.

When making appointments, please be as specific as possible to ensure proper time is given to your appointment. Should you need to cancel an appointment, please call as far in advance as possible so that an opportunity may be provided for other patients.

1st Appointment

To schedule the first appointment, please contact this office by telephone (407-740-0909) or email (CIRM@aol.com). A patient registration form and a medical release form will be sent to you. We request that your medical records be forwarded to this office for review prior to your initial visit. These medical records, especially previous infertility treatments, are very important in your evaluation.

At the first appointment, there may be additional forms to complete. Thereafter the new patient will meet members of the nursing staff as well as one or more of the physicians. A thorough medical and infertility history will be taken and a targeted physical examination may be performed. An educated opinion will be rendered with recommendations made. Every patient is also afforded the opportunity to review insurance benefits and potential costs for infertility treatment.

The Center for Reproductive Medicine is a subspecialty referral practice and primary care is not rendered. It is important for the patient to maintain contact with her primary care physician for routine maintenance and annual examinations. Primary care, including annual exams and screening (e.g. pap smears, breast exams and mammograms), is not provided at the Center.

After Hours

During evenings, nights and weekends, all telephone calls are routed through the answering service. The answering service is unable to schedule appointments; however, the on-call nurse or physician may be contacted depending on the nature of the call.

Emergencies

If a medical problem arises such that a patient must be seen immediately, every effort will be made either to see the patient in the office or in the emergency department at the appropriate hospital. All emergency calls received through the answering service will be forwarded to the appropriate person on call.

Weekend Appointments

We are available most Saturday and Sunday mornings to perform inseminations. Please call the office at 407-740-0909 for assistance in scheduling a weekend appointment.
